New Patient Paperwork
To ensure your first appointment goes as smoothly as possible, please complete your new patient paperwork prior to arriving at our radiation oncology center. For more information about your first visit, check out the Your First Appointment page.

- Assignment of benefits form
- HIPAA Patient Privacy Rights Notification
- Medical history
- Family history
- Current medications list
- Allergies
- Current symptoms

- American Urological Association (AUA) Symptom Score form
- Sexual health survey
- Authorization for release of private health information to caregivers
Patient Portal
Our cancer center uses Noona to share information with patients and allow patients to manage their personal health electronically at their own convenience. Existing patients may access their patient portal using the link below.
